Submit a Claim - Claim Instructions

To qualify for a payment from the Net Settlement Fund, you must submit a timely and valid Claim Form. The Claim Form must be submitted by email no later than May 27, 2022 or, if mailed, postmarked or received no later than May 27, 2022.
